Martin F. Wieschhaus, MD, ABFM, FAAFP

Dr. Martin Wieschhaus
is originally
from Missouri
and made his way
to South Bend
for residency
training at Saint
Joseph Regional
Medical Center.
He received his
undergraduate
training from
the University
of Missouri-Columbia
in chemical engineering.
Between his second
and third year
of medical education
at Columbia, he
completed a one
year fellowship
in Pathology.
Following graduation
from residency,
Dr. Wieschhaus
entered solo private
practice for four
years in South
Bend.
In 1993, Dr. Wieschhaus returned to our program as Associate Director and
Medical Director of the Family Practice Center. The following year he completed
a one-year fellowship at the National Institute for Program Director Development
in Kansas City, Mo. In 1998, he assumed the directorship of our residency
program.
Dr. Wieschhaus is a Diplomate of the American Board of Family Practice and
a Fellow in the American Academy of Family Physicians. He and his wife Rhonda
have five children.
John E. Blomstedt III, DO
Interim Director of Medical Education
Osteopathic Family Medicine Residency Program
Dr. John Blomstedt is originally from Nebraska. He completed is residency training at Saint Joseph Regional Medical Center in June, 2007. He received a Bachelor’s degree in Exercise Science from the University of Nebraska, Lincoln and completed his medical school training at Des Moines University Osteopathic Medical Center.
Dr. Blomstedt is a member of the American Osteopathic Association and the American College of Osteopathic Family Physicians. He and his wife Jessica have five children.
Kevin Ericson, MD, ABFM, FAAFP

After graduating
from Wheaton College
in Illinois, Dr.
Kevin Ericson
attended the University
of Wisconsin-Madison
Medical School.
He completed residency
training at St.
John’s Mercy Medical
Center in St.
Louis, Mo. and
then went on to
private practice
in Green Bay,
Wis., for four
years.
Pursuing a desire to teach, Dr. Ericson then took a position as a faculty
member at Eastern Maine Medical Center’s Family Medicine Residency program
in Bangor, Maine, where he stayed for four years.
Dr. Ericson joined
our program in
1997 and has completed
a Faculty Development
fellowship through
Duke University.
He maintains a
strong interest
in medical missions
and is the Director
of our International
Health Track.
He spent time
as a medical student
in Liberia working
in a mission hospital
and has served
on short-term
medical missions
to Guatemala.
He is a Diplomate of the American Board of Family Practice and a Fellow in the American Academy. Dr. Ericson and his wife, Lisa, have four children.
Julia Fashner, MD, ABFM
Associate Director
Dr. Julia Fashner graduated with a Bachelor’s degree in biology and chemistry from Ohio Northern University. She attended Wright State University, where she pursued a Master’s degree in biology and then her medical degree. During medical school, Dr. Fashner was involved with the Family Medicine Interest Group, including serving as Vice President during her third year. After completing residency at the Dayton Community Family Practice Residency Program in 2000, Dr. Fashner joined the faculty here in South Bend for five years. After a brief departure to her home state of Ohio, she returned in 2007 to continue her career in teaching residents.
Dr. Fashner completed the Primary Care Faculty Development Fellowship Program at Michigan State University in 2003. She participated in the research track and developed a project that compared histology and cytology results done at colposcopy. She presented a poster of the results at the Northeast Region Society of Teachers of Family Medicine meeting in 2003. Dr. Fashner has also served as co-author and mentor on several resident projects that were poster presentations at Michigan State University Family Medicine Research Day in 2005.
Dr. Fashner is a Diplomat of the American Board of Family Medicine. She has varied interest in family medicine including diabetes, women’s health, patient education and scholarly activity. Dr. Fashner has also been instrumental in bringing the Reach Out and Read to the Family Medicine Center. This program gives children a new book at well-child visits throughout their first five years of life.
Jennifer Ludwig, MD, ABFM, FAAFP
Associate Director
Family Medicine Residency Program
Dr. Jennifer Ludwig is a native of the South Bend
area and a graduate of the University of Notre
Dame and Indiana University School of Medicine.
She also is an alumnus of our residency program.
In 1999, Dr. Ludwig joined the faculty of our
program. She serves as the Site Director for Indiana
University School of Medicine's third year Family
Medicine Clerkship and oversees the program's
medical student rotations. She also serves as
Co-Chair of our Recruiting Committee. Dr. Ludwig
has a special interest in women's health care
and is an instructor of Advanced Life Support
in Obstetrics. Dr. Ludwig is a Diplomate of the
American Board of Family Medicine and a Fellow
in the American Academy of Family Physicians.
She completed a Family Medicine fellowship through
the National Institute for Program Director Development.
Jennifer and her husband David have three daughters and a son.
Theodore R.
Neumann, MD, ABFM, FAAFP
Associate Director
Family Medicine
Residency Program
Dr. Ted Neumann
grew up in Delavan,
Ill. He graduated
from Goshen College
and Southern Illinois
University School
of Medicine. Following
medical school,
he completed his
residency at our
program in 1980.
After residency,
Dr. Neumann entered
private practice
in southwestern
Michigan until
1986. At that
time, he returned
to our program
as Director until
1994 at which
time he became
an Associate Director.
Dr. Neumann serves as the Medical Director of the Visiting Nurse Association
and previously served as Interim Medical Director of the hospital’s Cardiac
Rehab unit. He directs the internal medicine, nursing home, home visit, and
cardiac rehab components of our residency program.
Dr. Neumann holds
a Certificate
of Added Qualifications
in Geriatrics.
He is a Diplomate
of the American
Board of Family
Practice and a
Fellow in the
American Academy
of Family Physicians.
Dr. Neumann’s
special interests
are internal medicine
and geriatrics.
He completed a
Faculty Development
fellowship at
Michigan State
University where
he focused on
a curriculum in
exercise stress
testing.
Stephen Simons,
MD, ABFM, FACSM
Associate Director
Director, Sports
Medicine Fellowship
Dr. Stephen Simons
is a native of
Goshen, Ind. He
attended Southern
Illinois University
School of Medicine.
Dr. Simons also
is an alumnus
of our residency
program. Following
residency, he
worked for the
University of
Notre Dame as
a university and
team physician.
Currently he serves
as the director
of our Sports
Medicine Fellowship.
Dr. Simons is very involved in the American College of Sports Medicine and
the American Medical Society for Sports Medicine. He enjoys serving on committees
for these organizations and interacting with fellowship directors as well
as nationally recognized experts in Sports Medicine. He is a volunteer physician
for the United States Olympic Committee and recently served as an athlete
physician at the Salt Lake City Winter Olympics.
Dr. Simons is a Diplomate of the American Board of Family Practice,
and a Fellow in
both the American
College of Sports
Medicine and the
American Academy
of Family Physicians.
He holds a Certificate
of Added Qualifications
in Sports Medicine.
Daniel Triezenberg,
MD, ABFM, FAAFP
Associate Director

Dr. Daniel Triezenberg
graduated from
Calvin College
with a major in
Human Communication.
His medical degree is from the University of Michigan Medical School, where he was elected to membership in Alpha Omega Alpha. During his Family Medicine residency at the Medical University of South Carolina in Charleston, he was Chief Resident for Unit Affairs. He is a Diplomate of the American Board of Family Practice and a Fellow of the American Academy of Family Physicians. He is married and has three children.
Dr. Triezenberg worked in private practice in
Big Rapids, MI from 1982-1999. During this time, he was involved with education of medical students, PA students, and FP residents. He participated in a statewide practice-based research network. Medical staff offices included CME chair and Chief of the Medical Staff.
Dr. Triezenberg joined the faculty of our residency program in 1999. He serves as Medical Director of the Family Medicine Center. He has completed a faculty development fellowship with the National Institute for Program Director Development.
Interest areas include Family Practice Maternity care, End-of-life care, faith and spirituality in medicine, and the physician-patient relationship. His research interests involve Practice-Based Research Networks and investigation of the Review of Systems. He is an advocate for Information Mastery skills for residents, Electronic Medical Records, and Idealized Design of Clinical Office Practice. International medical experiences have been in Guyana and Guatemala.
Dominic Vachon, M. Div., Ph.D.
Director of Behavioral Medicine and Caring Science Training
Dr. Dominic Vachon
graduated from
the University
of Notre Dame
in 1980 with bachelor
degrees in psychology
and philosophy.
In 1985 he received
a Master of Divinity
degree from the
University of
Notre Dame. In
1993 he received
a Ph.D. in Counseling
Psychology from
Loyola University
of Chicago. He
completed a predoctoral
internship at
Oaklawn Psychiatric
Hospital in Goshen,
Ind. and at the
Notre Dame Counseling
Center. He later
completed a Scientist-Practitioner
Postdoctoral Fellowship
at the University
of Notre Dame.
Dr. Vachon is the Director of Behavioral Medicine and Caring Science Training for the Family Medicine Residency Program. He is also an adjunct professor in the Theology Department at the University of Notre Dame and Clinical Assistant Professor for the Indiana University School of Medicine. Previously he was a staff psychologist at the University of Notre Dame Counseling Center and a Concurrent Assistant Professor in the Psychology Department for nine years and later an adjunct professor for the Psychology Department for seven years. He also specializes in Neurofeedback therapy. He has a private practice and previously worked for the Holy Cross Health System Corporate Office in the areas of organizational ethics and mission effectiveness.
Dr. Vachon conducts research in the areas of empathy and burnout in the helping professions, the psychology of caring, family caregiving, and the role of spirituality in helping others. He founded the Institute for the
Support and Study of the Caring Professions, a private consulting and educational organization that does workshops and consultation for healthcare and other organizations concerned about caring and stress in helping people.
Edward Sheridan, PharmD, BCPS, CDM, BC-ADM, AE-C
Director, Pharmacy Practice Residency Programs
Originally from Ohio, Dr. Edward Sheridan came to Indiana to study pharmacy at Butler University in Indianapolis, receiving his bachelor degree in pharmacy in 1995. While practicing as a community pharmacist, he enrolled in Butler University’s Doctor of Pharmacy program, completing the degree in 1996. Dr. Sheridan received his board certification in pharmacotherapy in 2002.
Because of his passion for teaching, Dr. Sheridan joined our program in 1996. While he formally precepts the family medicine residency’s elective pharmacotherapy rotation, pharmacotherapy principles are also well integrated longitudinally throughout the entirety of the residency program. He also serves as the pharmacy practice residency program director and is an Adjunct Assistant Professor of Pharmacy Practice at Butler University, as well as an Affiliate Assistant Professor of Pharmacy Practice at Purdue University, frequently taking students at the Family Medicine Center.; He was given Purdue University’s Pharmacy Preceptor of the Year award in 2004. He is the course director of pharmacology at Indiana University School of Medicine’s Notre Dame campus. Dr. Sheridan also assists in planning the curriculum for the American Society of Health System Pharmacy’s national midyear meeting.
In addition to his teaching roles, he also chairs the Family Medicine Center’s performance improvement team. He is the director of ambulatory pharmacy service department. This department carries forth the mission of Saint Joseph Regional Medical Center by providing underinsured and uninsured patients with access to medications.
